Job Overview:
Professional Home Care Associates (PHCA) is seeking a compassionate and attentive Home Health Aide (HHA) to assist patients with personal care and daily living tasks. The ideal candidate will provide essential support to patients recovering from illness or injury, helping them maintain their independence while following a care plan developed by licensed clinicians and supervised by a Registered Nurse (R.N.).

Key Responsibilities:

  • Assist patients with personal care tasks, including bathing, grooming, dressing, and toileting.

  • Follow individualized care plans established by licensed clinicians.

  • Work under the supervision of an R.N. to provide safe, high-quality care.

  • Assist with mobility, including transferring patients and using assistive devices.

  • Document care activities accurately and report any changes in the patient’s condition to the healthcare team.

Qualifications:

  • Valid Home Health Aide (HHA) certification in the state of California.

  • Experience in home care or personal care settings is preferred.

  • Ability to work independently and follow detailed care plans.

  • Compassionate attitude with strong communication and interpersonal skills.

  • Physical ability to assist patients with mobility and perform caregiving tasks.

Physical Requirements:

  • Ability to perform physical tasks such as lifting, standing, and assisting patients with mobility.

  • Must be able to travel to patients' homes as needed.
