Our services
Skilled Nursing – RN
Provides professional nursing care at home, including disease education, medication management, wound care, and monitoring of acute and chronic conditions. Nurses assist with post-operative care and ensure patients receive personalized support during their recovery.
Occupational Therapy
Helps patients regain independence in daily activities by teaching techniques and proper use of assistive devices. Occupational therapists support patients in adapting to physical challenges through equipment training and energy conservation strategies, allowing them to live more independently.
Medical Social Work
Our Medical Social Workers provide emotional support and help address practical concerns such as financial planning and accessing community resources. They work with patients and families to develop personalized strategies for overcoming challenges during recovery.
Physical Therapy
Focuses on improving mobility, strength, and balance, particularly after joint replacement surgeries. Physical therapists help patients restore their physical function and independence, prevent falls, and improve gait through personalized exercise programs.
Speech Therapy
Assists individuals with speech, language, and swallowing difficulties, often after a stroke or other medical condition. Speech therapists work to restore communication abilities and improve swallowing, enabling patients to communicate and eat safely.
Home Health Aide – HHA
Provides assistance with personal hygiene, such as bathing, dressing, and grooming, to ensure patients maintain their dignity and comfort at home. Home health aides offer support with activities of daily living, ensuring patients' needs are met in a caring and compassionate manner.

Acceptance to Service
Patients are accepted for treatment on the reasonable expectation that PHCA can meet the patient’s anticipated medical, nursing, rehabilitative, and social needs in the patient's place of residence in an appropriate and safe manner. PHCA has limitations to the following specialty services: PHCA does not accept patients with Tracheostomy tubes, patients requiring a Ventilator or Intravenous (IV) therapy, and Pediatric patients. PHCA only has availability for Speech Therapy services in the Livermore Branch territory.
