Job Overview:
Professional Home Care Associates (PHCA) is seeking a compassionate and experienced Medical Social Worker to join our home healthcare team. The ideal candidate will provide social and emotional support to patients and their families, helping them navigate healthcare systems, access community resources, and cope with the challenges of illness or recovery. This role involves evaluating patient needs, offering guidance on available services, and collaborating with the healthcare team to ensure comprehensive patient care.

Key Responsibilities:

  • Conduct assessments of patients’ social, emotional, and financial needs to develop care plans.

  • Provide counseling and support to patients and families in managing illness, recovery, and transitions of care.

  • Assist patients in accessing community resources, including transportation, Meals on Wheels, and medical equipment.

  • Facilitate communication between patients, families, and healthcare providers to ensure coordinated care.

  • Provide education and guidance on available healthcare programs, benefits, and services.

  • Maintain accurate and timely documentation in electronic health records (EHR).

  • Advocate for patients’ rights and ensure they receive appropriate care and services.

  • Stay current on community resources and best practices through professional development.

Qualifications:

  • Valid state license and certification as a Medical Social Worker required.

  • Previous experience in a healthcare or home care setting preferred.

  • Strong knowledge of healthcare systems, community resources, and patient care coordination.

  • Excellent communication, interpersonal, and counseling skills.

  • Ability to work independently and as part of a multidisciplinary team.

  • Proficiency in electronic health record (EHR) systems.

Physical Requirements:

  • Ability to travel to patients’ homes as needed.

  • Capacity to sit, stand, and engage in patient care tasks for extended periods.
