Job Overview:
Professional Home Care Associates (PHCA) is seeking a dedicated and compassionate Occupational Therapist to join our home healthcare team. The ideal candidate will provide in-home occupational therapy services, helping patients regain independence in their daily activities. This role involves evaluating patient needs, creating personalized treatment plans, and working collaboratively with other healthcare professionals to help patients improve their quality of life.
Key Responsibilities:
Conduct thorough occupational therapy assessments to evaluate patient abilities and limitations.
Develop and implement customized treatment plans to enhance patients’ ability to perform daily living activities (e.g., dressing, eating, bathing).
Recommend and train patients in the use of assistive devices and adaptive equipment.
Provide education on energy conservation techniques and home modifications to ensure a safe environment.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
Maintain accurate and timely documentation in electronic health records (EHR).
Stay current on best practices and innovations in occupational therapy through ongoing professional development.
Qualifications:
Valid state Occupational Therapist license required.
Experience in home healthcare or outpatient occupational therapy preferred.
Strong knowledge of adaptive equipment, rehabilitation techniques, and patient care protocols.
Excellent communication and interpersonal skills.
Ability to work independently and within a team.
Proficiency in electronic health record (EHR) systems.
Physical Requirements:
Ability to lift and assist patients as needed.
Capacity to stand, walk, and perform physical tasks for extended periods.
