Professional Home Care Associates (PHCA) is seeking a skilled and compassionate Physical Therapist to join our dedicated home healthcare team. The ideal candidate will provide in-home physical therapy services to help patients recover mobility, improve strength, and restore independence. This role involves evaluating patient needs, developing personalized treatment plans, and collaborating with other healthcare professionals to achieve optimal outcomes for our patients.
Key Responsibilities:
Conduct comprehensive physical therapy assessments to evaluate patient needs and goals.
Develop and implement individualized treatment plans to improve strength, mobility, balance, and coordination.
Provide therapeutic exercises and interventions to enhance patient recovery and prevent falls or injuries.
Monitor and document patient progress, adjusting treatment plans as necessary.
Educate patients and caregivers on home exercises and safety techniques to maintain and improve mobility.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and achieve the best possible patient outcomes.
Maintain accurate and timely documentation in electronic health records (EHR).
Stay current on best practices in physical therapy through ongoing professional development.
Qualifications:
Valid state Physical Therapist license required.
Previous experience in home healthcare or outpatient physical therapy is preferred.
Strong knowledge of therapeutic exercise programs and rehabilitation protocols.
Excellent communication, organizational, and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic health record (EHR) systems.
Physical Requirements:
Ability to lift and move patients using proper techniques.
Ability to stand, walk, and perform physical tasks for extended periods.
