Professional Home Care Associates (PHCA) is looking for a compassionate and skilled Speech Therapist to join our home healthcare team. The ideal candidate will provide in-home speech therapy services to help patients regain communication and swallowing abilities. This role involves assessing patient needs, developing individualized treatment plans, and working closely with other healthcare professionals to promote the best possible outcomes for patients.
Key Responsibilities:
Conduct comprehensive speech and language assessments to evaluate patient needs and abilities.
Develop and implement individualized therapy plans focused on improving speech, language, communication, and swallowing.
Provide exercises and techniques to help patients regain speech function and improve swallowing safety.
Educate patients and families on effective communication strategies and techniques for managing speech or swallowing disorders.
Monitor patient progress and adjust therapy plans as needed to ensure optimal outcomes.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and provide updates on patient progress.
Maintain accurate and timely documentation in electronic health records (EHR).
Stay current on best practices and innovations in speech therapy through ongoing professional development.
Qualifications:
Valid state Speech Therapist license and certification (CCC-SLP) required.
Previous experience in home healthcare or outpatient speech therapy preferred.
Strong knowledge of speech, language, and swallowing rehabilitation techniques.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic health record (EHR) systems.
Physical Requirements:
Ability to work with patients in home settings and provide necessary physical support during therapy.
Capacity to sit, stand, and engage in therapy tasks for extended periods.
